  10. my dying shall not be artificially prolonged under the 
  11. circumstances set forth in this declaration:

  14.     If at any time I should have an incurable injury, disease,
  15. or illness certified to be a terminal condition by two (2)
  16. physicians who have personally examined me, one (1) of whom
  17. shall be my attending physician, and the physicians  have
  18. determined that my death is imminent whether or not life-
  19. sustaining procedures are utilized and where the application of
  20. such procedures would serve only to artificially prolong the
  21. dying process, I direct that such procedures be withheld or
  22. withdrawn, and that I be permitted to die naturally with only
  23. the administration of medication, and the administration of
  24. food and water, and the performance of any medical procedure 
  25. that is necessary to provide comfort care or to alleviate pain.
  26. In the absence of my ability to give directions regarding the 
  27. use of such life-sustaining procedures, it is my intention that 
  28. this declaration shall be honored by my family and physician(s)
  29. as the final expression of my legal right to control my medical
  30. care or treatment. I am legally competent to make this
  31. declaration, and I understand its full import.
